i#1551 port to ARM: predicated SIMD/FP, part 2: table

Add predicated SIMD/FP instruction entries into the decoder tables.  There
are quite a few of these and they require several new layers of tables.

Modifies the generator script to improve PUW output and handle multimedia
encodings better.  Still, far too much manual work is required to create
these tables.

Adds decoding rules for the new tables.
